Here is what is needed for your first appointment:
- Referral from your Primary Care Physician
Our office requires a referral from your primary care physician to be sent to our office fax line at (707) 980-6692 or via the EPIC referral system.
- Call To Schedule Your Appointment
Call our office at (707) 980-6636 to schedule your initial appointment. We are open Monday through Friday, from 8:30 am to 5:00 pm. We are closed on weekends.
- Fill Out the New Patient Packet
All patients must fill out the New Patient Packet. You may have received this in the mail after scheduling your appointment, or you can also download it on this page. Please fill it out ahead of time and bring it with you to your first appointment.

- Bring Valid Identification and Insurance Cards to your Appointment
Please bring your state identification card as well as your medical insurance card(s). If you have multiple insurance coverages please bring all cards.
