New patients: what you need for your first visit

To become a new patient at INPM, ask your primary care physician to send a referral by fax to (707) 980-6692 or through EPIC. Then call (707) 980-6636 to schedule your first appointment, fill out the New Patient Packet before the visit, and bring a photo ID and all your insurance cards.

What do you need for your first appointment?

  1. A referral from your primary care physician. Our office requires a referral from your primary care physician, sent to our fax line at (707) 980-6692 or through the EPIC referral system. Our referral guide explains the steps.
  2. A call to schedule. Call our office at (707) 980-6636 and press 1 during business hours to schedule your initial appointment.
  3. The New Patient Packet. All patients fill out the New Patient Packet. You may receive it in the mail after scheduling, or download it below. Fill it out ahead of time and bring it with you.
  4. Your ID and insurance cards. Bring a state identification card and your medical insurance card. If you have more than one insurance plan, bring all your cards.

Where will you be seen?

INPM has two offices. Your appointment can be at either one, depending on the clinician and the day.

Can I make an appointment without a referral?

No. INPM requires a referral from your primary care physician before your first appointment. Your doctor can fax it to (707) 980-6692 or send it through the EPIC referral system.
